We are very pleased to announce that ASiT Medal Winner 2011: Miss Sreelakshmi Mallappa MBBS, MRCS(Eng), MRCS(Ed) from St Mark’s.

Sponsored by Ansell, ASiT’s most prestigious prize is awarded to the best presentation from a surgical trainee. This is selected from the highest scoring abstracts delivered as part of the ASiT Medal oral presentation session. The winner is presented with the ASiT Medal and invited to present their work in the ASiT Session at the Association of Surgeons of Great Britain and Ireland International Surgical Congress.

Pre-operative neutrophil lymphocyte ratio greater than 5 is a prognostic factor for recurrent colorectal cancer

Colorectal cancer is the second most common cause of cancer-related death in the UK. Various studies to date have established that the disease progression in cancer is not only determined by the local characteristics of the tumour itself but also by the systemic host response to the tumour.

A range of inflammation-based prognostic scores have been used in various studies in predicting survival in patients with primary operable cancer. Neutrophil lymphocyte ratio (NLR) is one such inflammation-based marker which has been shown to be associated with progression and metastasis of various tumours including colorectal cancer. Currently however, there is relative paucity of data regarding the effect of pre-operative NLR on disease-free survival in patients undergoing elective, potentially curative surgical resection for colorectal cancer.

The aim of the present study was to examine the predictive role of preoperative NLR in the disease-free survival following elective, potentially curative resection for colorectal cancer.

Conclusions:

  • In the current study, we demonstrated that elevated preoperative NLR is an independent prognostic factor predictive of CRC recurrence followingelective curative resection
  •  
  • NLR is a cost-effective, easily reproducible and widely available blood test which forms part of every patient’s routine pre-operative workup
     
  • NLR is a tumour stage independent biomarker which can be used as a well-defined therapeutic target for future clinical trials
     
  • Pre-operative NLR may also have a role in counselling patients undergoing elective, potentially curative resection of CRC about the possible outcomes
